   Leaving the Hospital
Your physician will determine when it is medially appropriate for you to be discharged from the hospital. He or she will discuss this with you in advance so that you can make transportation arrangements with family or friends.

Before you leave, you will receive written instructions from you physician about your care and medications at home. If someone is going to be caring for you at home, it’s a good idea to have that person present when you review your care and medication instructions with your doctor. The questions below may be used as a guide:

  Do you clearly understand your physician’s home care instructions?
  Do you have any dietary restrictions?
  Are there activities that you should or should not do?
  Are there special medications you need to take and do you understand how they are to be taken and any potential side effects or interactions with other medications you may be taking?
  Will you need ongoing therapy such as physical or rehabilitative therapy? Have you scheduled those appointments?
  Will you need a follow-up appointment with your physician and has that appointment been scheduled?
